Please help us by taking our UM Member Survey

We are hoping to get a better sense of the willingness of our members to meet as we continue to manage the challenges of the COVID Pandemic. As we see opportunities beginning to open up, we also know that our membership is often among the most vulnerable because of the number of associated underlying conditions our members have.

This survey should only take a few minutes, but it will provide invaluable information to our Board members to plan for the retaining of 2021 and plan for 2022. We are asking for your quick response so we can start to make decisions as soon as possible.
